LHINC Council Meeting Communique

The LHIN Collaborative (LHINC) Council held its fifth meeting on July 9, 2010. This communiqué provides an overview of the key messages stemming from that meeting.

The following projects were discussed by LHINC Council: Priority Setting/Decision Making Process; Transition Management: “Home-First” Approach; Engagement of Public Health in LHIN Processes; Capacity Planning; LHINC Evaluation Framework.

Resource Matching and Referral project broadens scope to include mental health and addictions

Resource Matching and Referral (RM&R) is an electronic information and referral system that matches patients/clients to the earliest available services and care setting that best meets the individual needs of the client. In the Toronto Central LHIN, RM&R is now supporting e-referrals from 14 acute and rehab/complex continuing care hospitals to 39 long-term care homes and the Toronto Central CCAC.
