Resource Matching and Referral project broadens scope to include mental health and addictions

Resource Matching and Referral (RM&R) is an electronic information and referral system that matches patients/clients to the earliest available services and care setting that best meets the individual needs of the client. In the Toronto Central LHIN, RM&R is now supporting e-referrals from 14 acute and rehab/complex continuing care hospitals to 39 long-term care homes and the Toronto Central CCAC.

The goal is to have 80 percent of health service provider sites in the Toronto Central LHIN managing referrals electronically using RM&R in the next three years. According to a recent newsletter, the LHIN is working with the mental health and addictions and community support service sectors on plans to use RM&R to refer clients to and from those agencies over the next few years, including geriatric psychiatry and supportive housing services.
