Connecting communities via Integrated Assessment Record

The Integrated Assessment Record (IAR) pilot project in the Erie St. Clair and Central West LHINs, which launched in August 2009, continues to build momentum. The project is currently focused on community-based mental health organizations, but new partners include Westover Treatment Centre, Hotel Dieu Grace Hospital and Leamington District Memorial Hospital from the Erie St. Clair LHIN. Several other LHINS have expressed interest, and a provincial steering committee has been formed and is making progress in their oversight role, including the release of an RFP for IAR software.

The IAR currently supports the Ontario Common Assessment of Need (OCAN) and the RAI MH assessments for Community Mental Health, In-Patient in Hospitals and Crisis Centres. Future plans include support for other assessments such as RAI-MDS 2.0 for Long Term Care Homes (LTCH).
